To be seen at Crescent Community Clinic you must come to the Clinic during business hours and complete our intake form at Crescent Community Clinic 5244 Commercial Way, Spring Hill, FL 34606. Plan at least one hour for application, intake and eligibility, and to receive your first appointment date.
Since new patients must meet federal poverty guidelines, please bring photo ID and proof of income for either the last year or the last few months to the appointment in order to qualify financially for services provided at the clinic.
We are NOT accepting any applications other than the one completed in person at the Clinic.
- Our Health Clinic provides screenings for adults ages 18 through 64 with chronic health conditions - arthritis, asthma, cancer, COPD, emphysema, diabetes, epilepsy, depression, heart diesase, hypertension, and obesity. When possible, physicians may refer patients to specialists for extended care. We will do our best to provide specialist care if available. No narcotics are on the premises nor will we write prescriptions for any narcotic medications.
- We are NOT accepting Dental Clinic appointments as of October 1, 2019.
- Our Mental Health Clinic provides psychotherapy for mental illnesses but not alcohol or drug abuse. Narcotics are not prescribed at the clinic.
